Clinical observation in the USA

By: Maaz Ghouri

Maaz Ghouri gives his experience and advice for junior doctors considering work in the USA

After the problems of Modernising Medical Careers, I was one of the doctors who left for the United States in pursuit of an American dream—little did I realise that it would change my perception of my career and my life altogether. I am an overseas doctor who came to the United Kingdom in 2003 hoping to train as a surgeon. The recent changes in training brought about by Modernising Medical Careers meant that many of my colleagues left for either the US, Australia, New Zealand, the Middle East, or for their respective home countries. I chose to move to the United States of America. Before starting a training programme in the US, it is highly recommended that you do a clinical observership, which is counted towards your United States Clinical Experience; having this under your belt increases the chances of getting into a residency programme of your choice.

Types of USCE; Externship, Observership, Electives

By: StepTaker

USCE = United States Clinical Experience, it’s a general term that covers any clinical experience in USA.

Externship is an organized training program designed by hospitals and medical institutes to train “externs” by familiarizing them with the US medical care system without allowing them to interfere with patients management.
You might be allowed to take patient history and perhaps examine patients and write patient notes under guidance.
The program usually follows certain rules likes timing, schedules, assignments, insurance liability requirements, etc.
Most externship programs do not pay externs and many of them do require fees to be paid by the trainee in order to enroll in the externship program for certain duration. The term hands-on clinical experience usually refers to externship. Because externship is thought to be more effective than observership in getting students and grads into the US health care system, many commercial companies took advantage of this and started offering Paid Externships for IMGs.

Electives: From Application to geting LoRs. The Definitive guide for Egyptians.

By: Ahmed Elkhanany

Al Salamo Alykom

I wanted to write this guide long ago, but I was too lazy to start doing so. But alhamd lellah, I finally got together to compile it into a big informative thread. I hope it helps, even if for a little bit, any one of the readers…

During my Sixth year, around May 2009, I started the quest to the electives. I knew about it when someone mentioned the word (USCE). Later on I kind of started gathering information, and eventually I got a comprehensive picture of what I should do by around July 2009. I got into Yale SoM (Heme/Onc) for 4 weeks in April to June 2010, and GWUH (Cardiology, and Heme/Onc) for 8 weeks in January to March 2011.

  • My Elective experience was ONLY in IM, so I’ll just focus on that…
  • PS. If anything needs any further details NOT covered in the guide, I’d be gladly helpful if I know it, since what I’ve written here is about 95% of what I know, and I forgot the rest. Find me at www.facebook.com/elkhanany.

Writing your medical CV

By: Karim Fikry, MD
PGY-2 resident, Boston,MA

Curriculum Vitae (CV) also known as a “resume” is the document in which you explain to people who you are.  The person who reads it may be the program director, a research Principal Investigator, an attending you want to follow, etc…  To have a good CV, there are certain areas that you need to work on starting from NOW if you are a medical student.  If you are an intern or a resident, it’s never too late, but you’ll have to do double the work to build a respectable CV.  Let’s go over the headings that should be present on your CV and look at tips on how to build it.
